It is the master document which will guide you through the entire mobilization process. We recommend that you print, complete, and rely on it as your "road map" to the mission field. For Brazil, Yellow fever is the only required
shot. Check with your local health Dept.
for availability. You must have the Yellow
immunization card for international travel.

Your tickets will be bought through
LINC-UP Missions. Reimbursement of
the ticket price is due at this time.
6. Send Visa Application 5 weeks prior to departure _______________
You must use the Brazilian Consulate
in whose jurisdiction you reside.
Florida residents – www.brazilmiami.org
Residents of MS, AL, GA, SC, NC, TN must
use the Brazil Consulate in Atlanta.
